Hello Dr.I was recently on business in China and met a CSW from South Africa at a nightclub. I had protected vaginal sex with her then she removed the condom and performed oral sex on me for about a minute or two until I ejaculated. I've read so many threads that say oral sex is low risk for HIV but my fear is that there is still a risk and this would be the lottery I win. I am also summizing after much research that since she was from South Africa the odds are much higher that she would have HIV. Of course when I call online STD testing centers they tell me that my incident was very high risk and there are many cases where HIV is spread through receiving oral sex. It's been 10 days since the incident and for the past 4 days I've been having every symptom imaginable which I'm sure is due to anxiety and lack of sleep. My question is do you think testing is neccessary based on this exposure? I am back in the US today and I was thinking about doing the PCR RNA/DNA for early detection for peace of mind, are these tests accurate? If I take this test and get a negative result would it be conclusive? I only ask this question because the test ceters say even if I got a negative result I would need to take an antibody test in 1-3 months to confirm, if thats the case what good is the PCR?

Thank you very much for your post and welcome to our forum.
Your sexual encounter, as described, did not involve any risk of HIV infection to you, even if the lady was HIV positive. The risk of contracting HIV through oral sex is negligible, mostly theoretical, and this applies to performing oral sex, not recieving oral sex. There have been no reported cases of anyone becoming infected through receiving oral sex. Bearing all this in mind, you do not even require testing. The only justification for you to get tested, would be for your peace of mind, and not on medical grounds. You can always have the PCR RNA test, which has a high level of reliability after 10 days, around 99%. I am sure that this will be negative in your case, and you could consider it conclusive, bearing in mind your no risk sexual encounter, without the need of having any further tests. However it is true that current guidelines still recommend a follow-up HIV Duo test after 4 weeks, but this would not be necessary in your case.
